About the Opportunity Medix is partnering with a client to staff a crucial Travel Physical Therapist role at their facility in Jacksonville, Indiana. This is a unique and rewarding opportunity within a Program of All-Inclusive Care for the Elderly ( PACE ), a comprehensive model designed to empower seniors to age in place, living safely and independently in their homes and communities. You will be joining a brand new PACE center , which functions like a "YMCA for adults," offering coordinated healthcare and supportive services in an active, community-focused setting. This is an exciting chance to help establish the center's workflow and patient load as the census grows. For travelers who enjoy the contract, there's potential for immediate future placements as the group expands into new states. Location: Jeffersonville, Indiana Facility Type: Outpatient (Program of All-Inclusive Care for the Elderly - PACE) Start Date: November 3rd Length of Assignment: 13 Weeks (Option for 6 months) State License Needed: Valid State Licensure as a Registered Physical Therapist (PT/DPT) Shift: Monday - Friday, 8:00 AM - 4:00/4:30 PM (5 eight-hour shifts). Call/Weekends: No on-call, weekends, or evening shifts. Pay: Competitive compensation packages, including rate ranges and possible stipends, will be thoroughly discussed with your dedicated Medix recruiter. Role & Responsibilities As the Physical Therapist, you will plan and administer physical therapy treatment programs to maintain and/or restore function, relieve pain, and prevent further disability for PACE participants. Your day-to-day duties will include: Assessment & Treatment: Completing evaluations, providing treatment, and ramping up to a target caseload of approximately 10 scheduled patients per day as the center census increases. Therapy Focus: The majority of treatments will focus on generalized strengthening, balance, and fall prevention , with the goal of getting participants up and moving. Coordination: Coordinating care with the Primary Care Physician (PCP) and participating in developing interdisciplinary care plans and care team meetings. Equipment Management: Assessing the need for, ordering, fitting, and educating participants on adaptive equipment and Durable Medical Equipment (DME) such as knee braces, walkers, wheelchairs, and shoes. Documentation: Completing all required documentation, including assessments, progress notes, and care plans, utilizing the IntusCare (PACE-specific) EHR system. More About the Setting Patient Population: Elderly participants (age 55) who are active and mobile enough to be eligible for the PACE program. Facility Amenities: You will be working in a new center equipped with a full gym featuring brand-new equipment, including parallel bars, bands, weights, and balls.
