Job SummaryBaptist Health, the region’s largest not-for-profit healthcare organization, is seeking a Social Worker to intervene with patients and families for psychosocial needs, support, and assistance in coping with chronic, acute, or terminal illnesses. The role involves providing consultation, discharge planning, education, counseling, advocacy, and referrals to meet emotional, social, and financial needs. The Social Worker will also provide crisis intervention, coordinate discharge plans, serve as a liaison with community resources, and address issues such as child or elder abuse, domestic violence, and bereavement.
