The CommuniCare Family of Companies owns/manages nursing and rehabilitation centers, specialty care centers and assisted living communities across multiple states.ResponsibilitiesAssume the administrative authority, responsibility, and accountability of implementing our medical services, policies, and procedures according to federal, state and local regulations; and current practice of post-acute and long-term care medicine.Coordinate medical care, including emergency treatment procedures, maintain effective liaison with attending physicians, and implement methods to keep the quality of care under constant surveillance.Provide necessary physician services for residents within the skilled nursing facilities and/or clinic environment.QualificationsM.D. or D.O. requiredGerontology certification preferredMD or DO license in State of IndianaABMS Board Certified in Internal Medicine, Family Practice, Geriatrics or Hospice/Palliative5+ years of experience working with geriatric or long-term care populationDemonstrated experience working with and educating NP’sPreferred QualificationsHealth care management/leadership experienceFamiliarity with CMS, Medicare Advantage, SNP Rules and RegulationsHCC and Risk Adjusted reimbursement methodologyInpatient/SNF utilization metrics and bed day managementState specific collaborative practice agreements and midlevel practitioner licensingKnowledge of EMRBenefitsAs a CommuniCare employee you will enjoy a competitive salary and PTO plans.
