Join to apply for the Vice President, Population Health role at South Side Healthy Community Organization1 day ago Be among the first 25 applicantsJoin to apply for the Vice President, Population Health role at South Side Healthy Community OrganizationTHIS POSITION HAS A TARGET START DATE OF OCTOBER 2025****Position OverviewThe Vice President (VP) of Population Health is a key executive leader responsible for developing, implementing, and overseeing all population health strategies and value-based care initiatives for the South Side Healthy Community Organization (SSHCO). This role embodies our unwavering commitment to transforming health and scaling new models of care by improving health outcomes for residents on the South Side of Chicago, enhancing access to care, and reducing costs for stakeholders. The VP will lead a multidisciplinary team and ensure ongoing programmatic excellence and strategic initiative implementation across the organization. They will also collaborate with a network of healthcare organizations, community partners, and external stakeholders to drive innovation and build health equity.Key ResponsibilitiesTo perform this job successfully, an individual must be able to perform each essential job function satisfactorily. Reasonable accommodation may be provided to enable individuals with disabilities to perform essential duties.Develop and execute a comprehensive population health strategy that aligns with SSHCO's mission to improve the health and well-being of our south side community. This includes ensuring that key strategic projects are translated into actionable, quantitative, and measurable plans and are aligned with strategic priorities.Oversee all aspects of programmatic design, work plan development, and implementation. This includes leading project management for innovative programs targeting chronic conditions, behavioral health, and maternal health, as well as initiatives addressing care coordination and social determinant needs.Lead efforts to enhance care coordination and member engagement, which includes reaching hard-to-reach members and connecting them to coordinated care through a variety of channels (text, telephone, in-person).Serve as the primary strategic leader for all population health data needs, ensuring that data is used to drive targeted, data-driven interventions and track outcomes. Partner with the Data & Analytics team to define analytical requirements and monitor performance.Serve as a primary liaison to partners, including FQHCs and Medicaid MCOs. Build and maintain strong relationships to support sustainability and value-based arrangements.Provide strategic direction and leadership to a multidisciplinary team, including nurses and community health workers (CHWs), to support clinical and social needs.Participate in long-range strategic planning and development to support financial sustainability initiatives.Please note this description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of this position. Duties, responsibilities, and activities may change at any time with or without notice.Knowledge, Skills, And AbilitiesMaster's degree in healthcare administration, public health, business, or a related field, or an equivalent combination of a bachelor's degree and relevant experience is required.A minimum of 10 years of progressive leadership experience in a similar role within the healthcare or public health industry. The ideal candidate will have a proven track record of developing and managing successful population health programs and value-based care initiatives.
