RiverSpring Living provides a full range of care solutions including nursing home care, managed long term care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.The RiverSpring Licensed Home Care Services Agency (LHCSA) provides comprehensive and personalized home healthcare services for older adults to maximize their independence at home.Under the direction of the Director of Nursing, the AL-Nurse Manager will coordinate healthcare and wellness programs, as well as personal care services for residents in our Assisted Living or Adult Care facilities. This role involves clinical assessments, oversight, and implementation of individualized service plans developed in collaboration with residents.Key ResponsibilitiesOversee the implementation of health-related services for the Assisted Living Residence (ALR) and Assisted Living Program (ALP).Participate in the assessment and screening of applicants to determine appropriateness for care.Assist in the development and implementation of individualized service plans (ISP) based on assessments.Conduct assessments and reassessments to identify residents' healthcare needs and adjust care plans as necessary.Document resident care and changes in status.Supervise program nurses and home health aides.QualificationsGraduate of an accredited school of Nursing (BSN)Current Registered Nurse New York State LicenseExperience in staff supervision and medication oversight in Assisted Living, Nursing Homes, or similar environmentsStrong management and supervisory skills, with experience in assessments, care planning, and reassessmentsExperience with Dementia and/or Alzheimer’s patients is a mustMinimum of 2 years of experience providing services to older adults in settings such as home care, residential care, adult care facilities, or rehabilitationCompensationAnnual salary: $115,000 - $125,000Eligible for a $15,000 sign-on bonus (less taxes)Benefits include health insurance, retirement plans, paid time off, and professional development opportunitiesIf you’re ready to advance your career and make a significant impact in the field of home health care, we would love to hear from you!Equal Opportunity Employer (EOE)
