Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager. Observes signs and symptoms and reports them to the R.N. Case Manager. Notes changes in physical or emotional status of clients. Promotes client and caregiver independence in self-careUtilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of dataFollows Agency policies regarding universal precautions, and safety guidelinesFulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services. Qualifications EMR UsedHours Per Week: 10 - 40 Qualifications: Current State RN License Holds a current Licensed Practical Nurse license in Ohio Has a minimum of one year of nursing experience Has current CPR certification Has excellent communication, organizational, and human relations skills Is able to function with minimal supervision Is able to manage multiple projects/changing priorities Is able to employ critical thinking skills Job Types: Per diem, Part-time, PRN, Full-time#MP123
