Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.SummaryWe are seeking a compassionate and skilled Nurse Practitioner to join our team in a hybrid Hospice and Palliative Care role. This position provides both consultative palliative care and hospice medical services, ensuring high-quality, patient- and family-centered care across home, facility, and inpatient settings. This role serves as an NP in accordance with BTBH organizational policies and procedures and applicable laws and regulations.ScheduleMonday - Friday.; 8:30am-5:00pm; HybridLocation:San Mateo CountyBi-lingual in Spanish, Cantonese, Mandarin preferred**Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.Work EnvironmentHybrid role with visits in home, facility, and inpatient settings Travel within service region required; valid driver’s license and reliable transportation needed Some evenings, weekends, and on-call coverage required Key ResponsibilitiesDeliver bedside assessments, pain and symptom management, and goals-of-care conversations for patients with serious or life-limiting illness Conduct hospice face-to-face visits and document eligibility in compliance with CMS regulations Collaborate with the interdisciplinary team to develop and update care plans Educate patients, families, caregivers, and community providers on hospice and palliative care Ensure accurate and timely documentation to support quality care and regulatory requirements Education/ExperienceBachelor’s degree and graduate of accredited Nurse Practitioner Program or Master’s in clinical field of nursing or clinical field related to nursingExperience in an acute care, home health, palliative or hospice setting preferredCertificates, Licenses, RegistrationsAdvanced practice Registered Nurse with active Nurse Practitioner licensePublic Health Nursing certification is highly desirableHPNA certification preferredPay Range$70.00 - $80.00 hourlyMission Hospice and Home Care is proud to serve our community as an equal employment opportunity employer.
