Nurse - Ambulatory Service

Position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. This is an OPEN AND CONTINUOUS ANNOUNCEMENT. This announcement will remain open until September 30, 2026 (or when positions filled). The initial cut-off date for referral of eligible applications will be November 17, 2025. Eligible applications received after that date will be referred at regular intervals, every Friday, or as additional vacancies occur on an as-needed basis until positions filled. The Nurse Coordinator serves as nursing supervisor during non-administrative tours and has designated authority, responsibility, and accountability for managing and directing employees, independently allocating human and materiel resources and making necessary changes in staffing due to tour-to-tour unplanned leave and/or changes in patient acuity. Functions or Scope of Assigned Duties: The Nurse III Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for the Primary Care Department and facility. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations. Responsible for the documented PACT outcomes at the program or service level. PACT program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.
