The RN will be responsible for patient assessments, developing personalized care plans, supervising support staff, and ensuring the highest standards of nursing practice in collaboration with the care team.Key ResponsibilitiesConduct comprehensive admissions and assessments to develop individualized care plans.Monitor and evaluate patients' conditions, updating care plans as needed.Provide education and guidance to patients, families, and caregivers regarding disease management and self-care.SuperviseHome Health Aides (HHA)andLicensed Practical Nurses (LPNs)to ensure quality and continuity of care.Coordinate with interdisciplinary team members, including OT, PT, MSW, and speech therapists, to optimize patient outcomes.Document care provided and report changes in patient condition to physicians and other healthcare personnel.QualificationsActive RN License - Connecticut (required)Associate Degree in Nursing (required);BSN preferred0-1 year of experience inhome health, case management, or patient careBilingual in English/RussianStrong assessment, communication, and leadership skillsApplication Process: Please submit your resume to or Call (949) 404-4671.
