With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home. Working on-site at a major medical center, the Home Care Planner is responsible for evaluating and establishing a plan of care for patients referred for home health services. The Home Care Planner will facilitate admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members.
