Family Practice/Primary Care Nurse Practitioner

Annual Salary: $140k-$160k (Varies depending on years of experience)Our client is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join their dynamic team at a Federally Qualified Health Center (FQHC) in Brooklyn. This is a full-time position serving a diverse patient population with a strong focus on community health, chronic disease management, preventive care, and health education.Key Responsibilities:Provide comprehensive primary care services to patients of all ages, including physical exams, chronic disease management, acute care, and preventive screenings.Diagnose and manage common and complex medical conditions in accordance with evidence-based practice and FQHC standards.Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, behavioral health specialists, and social workers.Participate in care coordination and quality improvement initiatives.Maintain accurate and timely electronic medical records (EMR) in compliance with organizational and regulatory requirements.Educate patients on wellness, prevention, and treatment plans.Support community outreach initiatives and contribute to the overall mission of improving access to high-quality healthcare.Qualifications:Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of New York.Minimum of 2 years of direct clinical experience as a provider in a primary care or community health setting.Experience working with underserved and diverse populations preferred.BLS certification required; additional certifications (e.g., DEA) as needed for scope of practice.Excellent interpersonal and communication skills.
