Nurse Practitioner (Adult/Geriatric)- Dutchess country, NYLocation: Dutchess country, NY ( specifically Poughkeepsie and Wappingers Falls) Job type: Full-Time or Part-Time Day Shift Only Shift: Monday - Friday (No nights or weekends)Job Summary & Responsibilities:Perform comprehensive health assessments including physical exams, medication reconciliation, and medical history reviewsDevelop and manage individualized care plans in coordination with the interdisciplinary teamDiagnose and manage chronic and acute conditions with appropriate interventions and referralsOrder and interpret diagnostic tests (labs, imaging, etc.)Prescribe medications and treatments in accordance with state and facility guidelinesParticipate in the initiation of chronic care management servicesEducate residents and their families on disease prevention, treatment plans, and self-care strategiesCollaborate in quality improvement initiatives to enhance care delivery and outcomesMaintain accurate and timely documentation in the electronic health record systemEducation, Licensure, and Experience:Master's Degree in Nursing from an accredited Nurse Practitioner programActive and unrestricted Nurse Practitioner license in the State of New YorkCurrent DEA license for prescribing medicationsEligible for Medicare and Medicaid billingPreferred: Minimum of 3 years of clinical experience, particularly in geriatrics or long-term carePreferred: Familiarity with eClinicalWorks (ECW) or similar EHR systemsBenefits:All standard benefitsFor more information please reach out to Valeria Moore: at or by Email: at
