Family Practice/Primary Care Nurse Practitioner

Thanks!-Full time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm-Home care or hospice experience preferred-Chronic disease management experience preferred-Must be willing to travel within the designated market, up to 1 hour from home-Must be willing to visit patients in their homes 4 days/week, 1 day remotePay flexible For this location, there will be opportunity for travel. Typically 1x a month and could involve driving 2 hours (maybe more) away, staying overnight, or spending up to a week conducting visits.
