A respected, mission-driven healthcare organization is growing its hospice team in the Fargo area and is seeking a Hospice Nurse Practitioner to join their collaborative interdisciplinary team.Position Highlights:Opportunity to work in a supportive and collaborative cultureHospice is a key focus area within the broader health services providedStrong interdisciplinary approach, with NPs as active contributors to patient care planning alongside physicians Key Responsibilities: Conduct face-to-face visits and comprehensive assessments to ensure hospice benefit eligibilityProvide symptom management and end-of-life care to patients in home settings, assisted living facilities, and skilled nursing facilitiesUtilize telehealth to support rural or hard-to-reach patients as neededCollaborate with physicians, nurses, social workers, and other team members to deliver high-quality hospice careEducate patients, families, and staff on disease progression and treatment goalsProvide leadership in the application of advanced nursing practices Territory & Schedule: Average NP caseload: 5 patients/dayHospice census across the organization: 400+Schedule: Monday Friday, 8:00 AM 5:00 PM Qualifications: Licensed Nurse Practitioner (NP) in North Dakota and Minnesota (or eligible)DEA licensureCurrent driver s license and auto insuranceHospice NP experience strongly preferredAcceptable backgrounds include experience with geriatrics, palliative care, internal medicine, SNF, or related patient populationsPrior RN hospice experience a plus Compensation & Benefits: Salary range: $115,000 $140,000 (depending on experience27 days PTO (personal, vacation, sick, and holidays)License fee reimbursementDEA fee reimbursementUp to 5 additional days off and $2,000 annually for continuing educationMedical, dental, vision401(k) retirement plan
