Family Practice/Primary Care Nurse Practitioner

The ideal candidate will be passionate about serving diverse populations, promoting health equity, and working collaboratively within a multidisciplinary team.Key Responsibilities:Provide comprehensive primary care services to individuals and families across the lifespan.Conduct health assessments, diagnose and treat acute and chronic conditions.Promote preventive care, health education, and wellness strategies.Collaborate with physicians, nurses, social workers, and other healthcare professionals.Maintain accurate and timely documentation in the electronic health record (EHR).Participate in quality improvement initiatives and community outreach programs.Requirements:Current and unrestricted FNP license in New York.Minimum of 1 year of clinical experience preferred (new grads welcome to apply).Strong communication and interpersonal skills.Commitment to culturally competent care and community health principles.
