The team includes physicians, nurse practitioners, nurses, care specialists, and support staff all united by a shared mission to improve patients lives.They treat a wide range of needs from managing chronic conditions and addressing urgent medical issues to connecting patients with behavioral health support and social services because they believe health requires care for the whole person.Job Title & Role DescriptionThe Nurse Practitioner (NP) is responsible for providing high-quality, patient-centered, comprehensive primary care. Using a proprietary facilitated virtual care platform, the NP delivers care that improves outcomes and reduces costs for patients with complex diagnoses and multiple chronic conditions.This role is primarily virtual, with periodic or as-needed in-person visits for care transitions, annual wellness visits, risk adjustments, urgent visits, and other primary care needs. The NP works closely with a multidisciplinary team to provide whole-person care directly in patients homes.Key ResponsibilitiesTriage, diagnose, and treat patients in person or via the virtual care platformServe as a day-to-day clinical leader, providing decision support and guiding the multidisciplinary team under the direction of the Medical DirectorConduct primary care visits, urgent visits, and annual wellness exams, ensuring all appropriate screenings and quality measures are addressedRefer patients to behavioral health and specialty services as appropriateCoordinate care transitions to prevent hospital readmissions by collaborating with PCPs, hospitalists, and SNF providersParticipate in the on-call provider rotation for the 24/7 patient hotline, including after-hours and weekends as scheduledFoster a collaborative working environment by contributing to daily huddles, team meetings, and patient care discussionsMonitor and respond to remote patient data collected via monitoring devicesPerform other duties as assignedKnowledge, Skills & AbilitiesPerform and document accurate and thorough risk assessments for proper condition documentationExpertise in managing complex, high-acuity patientsExperience working with Medicare and Medicaid populations in inpatient, outpatient, or home care settingsProficiency in motivational interviewing and trauma-informed care (preferred)Excellent written, computer, and verbal communication skills with the ability to work effectively with the care teamDedication to health equity and improving access to careStrong organizational and leadership skillsQualificationsMinimum 5 years of experience as a provider, including at least 3 years in primary care or a related settingValid driver s license and auto liability insuranceWillingness and ability to travel as needed (primarily virtual role)Experience in urgent/emergent care settings is a plusEqual Opportunity Employer: The hiring organization is committed to attracting, retaining, and supporting a diverse and inclusive workforce.
