Psychiatric - Mental Health Nurse Practitioner

Annual Salary: $135k-$185k (varies depending on years of experience)A community-focused behavioral health organization is seeking a dedicated Psychiatric Nurse Practitioner to provide psychiatric care and medication management to adult men residing in transitional housing.Key Responsibilities:Conduct psychiatric evaluations and manage medication regimens for adult male clients with complex behavioral health needs.Support treatment adherence and coordinate care, including follow-up appointments and referrals.Collaborate with shelter staff and case managers to ensure continuity of care and crisis intervention.Provide hands-on support to clients, including motivational engagement and psychoeducation.Maintain accurate documentation and contribute to care planning in coordination with a collaborating psychiatrist.Travel between two fixed shelter locations weekly; mileage and tolls reimbursed.Requirements:Licensed Psychiatric Nurse Practitioner (PMHNP) in New York State.Minimum of 2 years experience working with adults with serious mental illness.Experience in shelter-based care, ACT/IMT teams, or similar community mental health settings strongly preferred.Must have access to a personal vehicle and be comfortable traveling between two boroughs.Strong interpersonal skills and a commitment to working with underserved populations.
