The Practice San Bernardino Health Center - San Bernardino, California Adelanto Community Health Center - Adelanto, California Ontario Health Center - Ontario, California A Federally Qualified Health Center.Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker.Facility hours for this location are Mon-Fri 8am-5pm.Good mix of patients including peds.15-18 patients per day per provider.Point of Care testing available at site; minimal labs.
