Geriatrics Nurse Practitioner

Job Title: Adult / Gerontology Nurse PractitionerLocation: New York, NYShift: Full & Part-time available Responsibilities: Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation. Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident Order & interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning Participate in care initiation for our chronic care management services. Requirements: Master's degree in nursing and graduate of an accredited Nurse Practitioner program Unrestricted New York state license as an ANP or AGNP Unrestricted DEA Certification Experience working with geriatric population and/or long-term care.
