Board Certified Nurse Practitioner (NP)2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, Palliative Care, and/or Home HealthSenior Medicare patients with complex needs to prevent hospitalization readmissions and reduce avoidable utilization, conduct Annual Wellness Visits, risk assessments, medication management, manage chronic conditions and coordinate care across specialties75% in clinic and 25% in home visitsMust have active CA license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and preferred to have AWV exp.Mon-Fri 8a-5p8-10 patients per daySalary based on experience $145-165K with plenty of great benefits and morePlease refer to Job ID.
