This role provides comprehensive primary care to older adult participants in a variety of settings, including the clinic, patients homes, and affiliated long-term care facilities. The Nurse Practitioner collaborates closely with physicians and the broader care team to ensure the highest quality of care, focusing on both acute and chronic conditions. Visits nursing facilities to address acute or chronic conditions when participants are unable to visit the clinic. Conducts home visits for participants who are homebound due to acute or chronic health concerns. Completes initial assessments for all new participants and conducts quarterly evaluations throughout their first year of enrollment.
