Advanced Practice Provider (APP) In-Home Chronic Care County, Pierce County & Seattle WA About the Role: We re seeking Advanced Practice Providers to join a nationally recognized leader in in-home care for patients with chronic kidney disease and end-stage renal disease. This innovative model combines technology-enabled care management, medication review, and patient education to improve outcomes, reduce costs, and expand access to high-quality, evidence-based care.
