Job Title: Nurse Practitioner Primary Care (School-Based / Community Health)Location: San Jose, CAStart Date: 10/20/2025 (or as soon as credentialed)Contract Length: 6 months (potential for extension)Schedule: Monday Friday, 8:00am 5:00pm (40 hours/week)Employment Type: Locum Tenens W2 Position Overview We are seeking a dedicated Nurse Practitioner to join a community health organization in San Jose, CA. The provider will work in a school-based setting serving both students and members of the local community. This is a full-time, Monday through Friday role focused on providing primary care services to patients of all ages. Responsibilities Provide comprehensive primary care to patients across the lifespan.Support chronic disease management and assist in meeting HEDIS quality measures.Collaborate with multidisciplinary staff to ensure high-quality patient care.Maintain accurate and timely charting using Epic EMR.Participate in a full week of mandatory orientation prior to starting. Details Patient Volume: 21 patients per day (21+ preferred)Patient Population: All ages (pediatric through adult)Setting: Outpatient / school-based primary careSupport Staff: On-site clinical and administrative supportOrientation: One full week (Monday Friday, required attendance)Credentialing Timeframe: 30 days or less Requirements Active California NP license (required)Board Certified Nurse PractitionerBLS certification (required)Experience with Epic EMR preferredMust be comfortable working with children and adultsWillingness to commit to the full 6-month contract Additional Notes BACH s needs may vary across sites, including school-based, brick-and-mortar, and mobile clinics.
