No Additional cases: Ortho, general surgery, scopes, dental, uro SUBMISSION REQUIREMENTS : AANA Board Certified - Required BLS, ACLS, & PALS - Required MI license at time of submission - Required Experienced CRNA with regional block and OB experience - Required Providers MUST participate in MI Medicaid/CHAMPS - required at time of presentation Clean background for quick credentialing - Required Please list availability at time of name clear COVID vaccination - Required at time of name clear
