Hospitalist Nurse Practitioner

As a TeamHealth post-acute care clinician, you will provide primary care coverage for patients in our skilled nursing facility Monday to Friday.TeamHealth provides support for clinicians by creating a collaborative work environment through the cohesive GehriMed EMR system. In clinical situations, you have a responsive back-office team to offer additional support. Clinicians also have unlimited access to training materials and robust CME access through TeamHealth's employee portal.Key ResponsibilitiesPerform comprehensive patient assessments, including medical histories and physical examsDevelop, implement, and adjust individualized care plans based on patient needsMonitor progress and coordinate care to optimize outcomesCollaborate closely with physicians, nursing staff, and other healthcare professionals in a team approachEducate patients and families on health management and preventive strategiesMaintain accurate, timely medical records in compliance with regulationsQualificationsCurrent NP or PA license (State of New York) and DEAExperience in post-acute, acute, emergency medicine, or clinic settings preferred but not required; new grads welcome to applyStrong clinical foundation and knowledge of healthcare regulatory standardsExcellent communication and interpersonal skillsCollaborative, team-oriented approach with a positive outlookAble to commute, relocate, and provide in person rounding in Amsterdam, NYExpected compensation is estimated salary range of $130,760 to $154,911 annually with no cap on productivity income potential.
