Geriatric Care Management Assistant - Chicagoland | Extremely Flexible ScheduleLocation: Chicagoland Area, North/Northwest SuburbsJob Type: Part-time / Full-time / FlexibleJOB DESCRIPTIONWe are seeking a professional, compassionate Geriatric Care Management Assistant to support multiple older adult clients throughout the Chicagoland area, working under the direct supervision of our professional aging life care management team within a growing private care management practice.This role is ideal for a highly flexible, reliable professional who enjoys working with older adults, values relationship-based support, and is interested in dementia care "” without providing hands-on personal care.Assignments May Include:Social visits and companionshipAccompanying clients to medical and personal appointmentsGoing on errands with/without clientAccompanying clients to community outingsMonitoring client well-being and safetyEmergency backup and short-notice coverageCommunication and coordination support for clients and familiesCustomized tasks individualized to clients needsThis position does not include hands-on personal care, medical care, or nursing services.Responsibilities:Follow individualized care plans developed by the supervising care managerProvide calm, respectful support to clients with memory challengesObserve and report changes in client condition to the care managerSafely transport clients to appointments and activitiesComplete simple visit notes and basic documentationCommunicate updates promptly and professionallyMaintain strict confidentiality and professional boundariesUse basic computer and mobile applications for scheduling and documentationRequirements:Experience working with older adults (professional or personal)Strong interest in dementia care and willingness to learnReliable personal vehicle and valid driver's licenseWillingness to travel throughout the Chicagoland areaFluent in EnglishExtremely flexible availability, including evenings, weekends, and short-notice assignmentsComfortable with basic computer skills and simple administrative tasksDependable, punctual, and professional demeanorClinical licenses (CNA, HHA, nursing) are welcomed but not required for this role.What We Offer:Highly flexible scheduling with varied assignmentsPaid travel timeOngoing mentorship from a professional care managerDementia-focused trainingProfessional growth opportunitiesSupportive environment within a growing private care management practiceCompetitive pay based on experienceJob Types: Full-time, Part-timePay: $ $25.00 per hourExpected hours: 20 - 40 per weekBenefits:Flexible schedulePaid orientationPaid trainingWork Location: On the road
