We are looking to collaborate with professional patient placement agencies to help connect patients with the right care settings.Responsibilities:Identify and refer potential patients to our facility or home care servicesScreen and assess patient needs to ensure proper placementMaintain clear communication with our admissions teamProvide accurate patient documentation and necessary referral informationFollow compliance and confidentiality standardsRequirements:Experience in patient placement or care coordinationKnowledge of assisted living, home care, or long-term care servicesStrong communication and organizational skillsProfessional network of hospitals, discharge planners, or social workersCompensation for this partnership will be based on a commission structure. You will receive a set fee or percentage for each patient successfully placed at our facility/home care service. The more patients you refer who are admitted, the higher your total earnings.
