Nurse Practitioner - Annual Health Assessments Salem, Oregon (Clinic + In-Home within 30-Mile Radius)Full-Time M-F 8:00 AM-5:00 PM PST Position Overview Permanent opportunity due to organizational growthPrimary Duties:Perform Annual Wellness Exams and health risk assessments for senior Medicare patientsDeliver care in clinic settings and in-home visitsFocus on high-risk patients, chronic condition management, and post-discharge supportCollaborate with interdisciplinary team (IDT) - social workers, case managers, etc.Perform HCC coding, care coordination, counseling, screenings, and discharge planningExpected patient volume:10-12/day in clinic5-6/day in-homeCharting due within 72 hours Ideal Candidate Passionate about serving underserved/Medicare populationsStrong commitment to preventative, holistic, value-based careExcellent communication and patient education skillsComfortable with both clinic and home visits Qualifications Current Oregon NP licenseFNP, Adult, Gerontology, or Palliative Care certification (ANCC/AANP)2+ years of NP experience in primary care or similar fieldsHome health or acute care experience a plusDEA License + Prescriptive Authority (can be reimbursed if obtained post-hire)CPR/BLS CertificationValid Oregon Driver's LicenseEMR proficiency (client uses their own internal system) Compensation & Benefits Annual Salary: $125,000 - $150,000 (based on experience)Benefits:Comprehensive medical, dental, vision401(k) with company matchPaid time off + parental leavePhone/internet reimbursementWellness support (BetterHelp, monthly reimbursement)Life & AD&D insurance (100% employer-paid)Optional coverage: disability, critical illness, accidentMalpractice insurance providedLicense and DEA reimbursementCE credits and professional development fundsCollaborative NP community & mentorship Support & Culture Backed by a strong care coordination teamEmphasis on teamwork, open communication, and continuous improvementEncourages innovation in care delivery and community-based care Interested?RecruitWell can schedule a call to connect you directly with the hiring organization
