Family Practice/Primary Care Nurse Practitioner

Position Overview: We are seeking an experienced Primary Care Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a collaborative, team-based primary care model. This role focuses exclusively on comprehensive primary care for an established patient population, including preventive care, acute visits, and chronic disease management.The APP functions as a licensed, independent provider in accordance with New York State regulations and organizational protocols, exercising sound clinical judgment and evidence-based practice. This position does not include specialty or surgical services. Primary Job Duties: Clinical Care Provide comprehensive primary care services including preventive care, wellness visits, acute illness evaluation, and chronic disease managementPerform complete and focused histories and physical examinationsDiagnose and manage common primary care conditionsPrescribe medications, therapies, and treatments within scope of practiceOrder, interpret, and follow up on labs, imaging, and diagnostic testsDevelop and monitor individualized care plansEmphasize preventive care, lifestyle modification, and patient education
