Geriatrics Nurse Practitioner

The Nurse Practitioner (NP) plays a vital role in delivering high-quality, patient-centered primary care to older adults enrolled in a PACE (Program of All-Inclusive Care for the Elderly) model. This position provides care both in a clinic-based setting and, as needed, in participants homes or partner facilities.As a core member of the Interdisciplinary Team (IDT), the NP collaborates closely with physicians, social workers, therapists, dietitians, and other care team members to ensure seamless, coordinated care.
