Role OverviewSchedule: 5 a week within Monday to FridayStart off at 10ppd and grow to 15ppd Provide patient care in a skilled nursing facility by conducting rounds in their short-term unitsConduct documentation at home with provided iPad laptop within 48 hours of seeing patientsCommute to 1 post-acute care facility per day within the Philadelphia Region Key ResponsibilitiesConduct psychiatric evaluations and assessmentsCollaborate with facility staff to enhance patient careDevelop and implement treatment plans for patients with a mental health diagnosisPrescribe and manage medicationsEnsure timely and accurate documentationRequirementsHold a valid full PMHNP licenseActive NY licenseActive DEA preferred but not requiredExperience in healthcare or long-term care is preferred but not requireCompensation is fee for service (FFS).
