Family Practice/Primary Care Nurse Practitioner

We are seeking acompassionate and experienced Nurse Practitioner to join our team in Mishawaka,Indiana. This full-time role works closely with clinical staff and reports tothe Site Medical Director and Site Operations Director, providing high-quality,patient-centered outpatient care within a Patient-Centered Medical Home (PCMH)model. Responsibilities: Provide comprehensiveoutpatient primary care and manage patient panels to ensure continuity of care Develop and implementeffective patient care plans, including referrals as appropriate Perform periodic healthassessments and provide patient education Maintain timely, accurate,and compliant clinical documentation Complete a minimum of 2,946outpatient visits annually Participate in qualityassurance activities, clinical meetings, and performance audits Support quality improvementinitiatives and clinical protocols Participate in communityoutreach, education, and student precepting as assigned Requirements: Master's Degree in Nursingfrom an accredited Nurse Practitioner program Active Indiana RN licenseand Nurse Practitioner certification Prescriptive authority withactive DEA registration Current CPR certification 1-3 years of NursePractitioner experience or significant RN experience in ER or ICU Compensation & Benefits: Competitivecompensation.
