Nurse Practitioner Long-Term Care New HampshireWe are seeking a Nurse Practitioner to join a facility-based medical team providing comprehensive primary and preventive care to patients residing in skilled nursing and long-term care facilities. This role offers the opportunity to make a meaningful impact on the health and well-being of the elderly population while working collaboratively within an interdisciplinary care team.ResponsibilitiesProvide primary and preventive care to long-term care residentsManage residents with acute changes in conditionPerform advanced care planning and chronic care managementPrescribe medications, including controlled substances, in accordance with NH lawOrder, review, and follow up on diagnostic testing and treatmentsCommunicate regularly with nursing facility staff regarding patient condition and care plansCollaborate with physicians and interdisciplinary care teamsScheduleMonday Friday, 8:00 AM 5:00 PM EST4 days per week (schedule coordinated with manager)QualificationsCurrent, unrestricted Nurse Practitioner license in New HampshireBoard Certified NPActive DEA licenseBackground in adult, family, or geriatric medicineExperience in a long-term care or skilled nursing setting preferredProficiency with EMR systemsStrong clinical judgment managing complex, chronic conditionsBenefitsContinuing education creditsRecertification reimbursementMalpractice insuranceHealth, dental, and vision insurancePaid time off
