About the Role In this role, you will play a vital part in delivering compassionate, effective care within post-acute care communities. You will manage a reasonable patient census and work closely with interdisciplinary teams to support optimal patient outcomes and continuity of care. Key Responsibilities Provide comprehensive primary care services to a patient panel of 15-18 patients per day Collaborate with interdisciplinary teams to develop and implement effective care plans Ensure smooth transitions of care with hospice, palliative care, and home care agencies Maintain accurate and timely documentation in the Gehrimed EMR system Qualifications Graduate degree as a Nurse Practitioner or Physician Assistant Active and unrestricted NP or PA license in New York DEA registration Strong clinical assessment and diagnostic skills Excellent communication and interpersonal abilities Experience in post-acute care preferred, but not required (training available) Compensation Estimated compensation range: $130,760 - $154,911 annually No cap on productivity-based income potential
