Nurse Practitioner / Physician Assistant - Primary Care Location: West Sacramento, CAJob Type: Full-TimeSchedule: Monday-Friday Day ShiftSetting: Community Health / Outpatient Clinic Responsibilities : Provide comprehensive primary care for acute and chronic conditionsConduct patient evaluations, diagnoses, and treatment planningDeliver preventive care, health education, and patient counselingCollaborate closely with medical leadership and care teamsManage a steady, structured patient volume with dedicated admin timeUtilize telehealth for a portion of daily visits Important Note: Patient Volume & Workflow ExpectationsAverage of 3 patients per hour (20-minute visits)Includes two telehealth visits per dayDedicated administrative time at the end of each dayChart completion required within 72 hoursAbility to efficiently manage the in-basket, including patient messages, refills, and lab resultsEPIC EMR experience highly preferred Qualifications : Active CA NP or PA license with board certificationDEA & BLS certification1-2 years of outpatient, community health, or FQHC experienceComfort working with multicultural and multilingual populationsExperience with EPIC EMR strongly preferredBilingual skills a plus (Spanish or other languages) Benefits Predictable weekday schedule (no nights or weekends)Average of 3 patients per hour with admin time built inCompetitive benefits including retirement match, health coverage, and generous PTOSupportive, team-based environment focused on access and quality careIf you're interested in learning more about this role, I'd be happy to connect.
