About the Role Position: Primary Care Nurse Practitioner Model: New primary care initiative focused on chronic disease management for a complex adult population Setting: Primarily in-person clinic-based care across several local sites Some flexibility for remote charting (half days as needed) Field-based presence across multiple clinics This is not a traditional dialysis or specialty NP role it is true primary care delivery within a new, high-visibility model.
