Ref #: C-OT-Profession: Occupational TherapyLocation: Fremont, CAType: Contract/Travel - 13 weeksStart Date: ASAPSchedule: Sun-Sat, 08:00 AM-04:30 PMRate: Travel: $2,648/40 hrs; Local: $2,608/40 hrsMinimum Requirements:Active, valid Occupational Therapy state licenseCurrent CPR and BLS certification (AHA or Red Cross, with AED)Primary Responsibilities:Evaluate patients' physical, cognitive, and environmental factors to identify functional limitations and establish goals.Develop individualized treatment plans in collaboration with patients and the interdisciplinary team.Implement evidence-based therapeutic interventions to improve activities of daily living (ADLs), instrumental ADLs, and independence.Recommend, train in, and fit adaptive equipment and assistive devices.Instruct and practice daily living tasks to build patient skills and safety.Educate patients and families on condition management, safety, and strategies to maximize independence.Monitor, document, and adjust treatment plans based on patient progress and outcomes.Maintain accurate, timely clinical documentation (evaluations, treatment notes, discharge summaries).Coordinate care and communicate with physicians, therapists, case managers, and other providers to support discharge and community/work reintegration.Participate in continuing education and stay current with occupational therapy best practices.Perform other duties as assigned by management.
