Job Title: Registered Nurse (RN) LTC / Rehab Department: NursingReports To: Director of Nursing / Nurse Manager Job Summary The Registered Nurse (RN) provides professional nursing care to residents in a long-term care and rehabilitation setting. The RN is responsible for assessment, care planning, medication administration, supervision of nursing staff, and coordination of resident care to promote optimal health outcomes. Essential Duties & Responsibilities Assess resident condition and develop, implement, and evaluate care plansAdminister medications and treatments per physician ordersMonitor resident status and respond to changes in conditionSupervise and support LPNs and CNAsCoordinate care with physicians, therapists, and interdisciplinary team membersPerform wound care, IV therapy, and other skilled nursing proceduresEnsure accurate and timely documentation in the medical recordParticipate in admissions, discharges, and care conferencesEducate residents and families on care plans and treatmentsEnsure compliance with facility policies, infection control, and regulatory requirementsRespond to emergencies and provide appropriate interventions Qualifications Current RN license in good standingGraduate of an accredited nursing programLTC or rehab experience preferred (new grads may be considered)Strong clinical, leadership, and communication skillsAbility to multitask and work effectively in a fast-paced environment
