With easy access to Philadelphia and a charming downtown, it's a great place for professionals looking to make an impact in healthcare and community wellbeing.Salary and BenefitsSalary range is $70,000 to $85,000 per year, depending on experienceHealth Insurance Amerihealth (Employee covers 70%)Dental Horizon Dental Total CareVision InsuranceVacation: 3 weeks (120 hours)Holidays: 8 days annuallyPersonal Days: 2 days annually (16 hours)Sick Time: 5 days accrued annually (40 hours)Bereavement: 5 days (40 hours) for immediate familyLife Insurance 3x annual salary up to $250,000Credit Union MembershipTuition Reimbursement ProgramDirect DepositFree Parking FacilitiesSupplemental Life, AD&D, Critical Illness and Accident InsuranceTemporary Disability: NJ State PlanLong-Term Disability: 60% of salary up to age 65 (Own occupation; 1st of the month after 1 year)Tax-Sheltered Annuity: 403(b)(After one year of employment 50% match, up to 3% of annual salary)ResponsibilitiesMonday - Friday schedule 8:30AM 5:00PMDevelop and implement the strategic vision, goals, and operational plans for the Sickle Cell Disease (SCD) programCoordinate and manage comprehensive patient care across inpatient and outpatient settingsServe as liaison to ensure integrated, person-centered care for individuals living with SCDLead a multidisciplinary pilot team including hematology, pain management, social work, and case managementBuild community partnerships and collaborate with public health agencies, advocacy groups, and donorsOversee community engagement initiatives and create educational outreach materials in partnership with marketingAnalyze data, track program metrics, and submit timely reports to organizational leadership and grantorsEnsure program compliance with all regulatory and funding requirementsSupervise the SCD Care Coordinator/Case Manager and Community Health WorkerMaintain flexibility in working hours, including occasional evenings and weekends as neededPerform additional duties as assignedRequirementsBachelor's degree in social work, public health, or related field requiredMinimum 2 years of experience in chronic disease management, social work (MSW preferred), hematology, or public healthDemonstrated experience leading multidisciplinary teams or community health programsStrong project management, leadership, and communication skillsAbility to work independently while maintaining team alignmentProficient in data analysis and outcome reportingDeep commitment to improving quality of life for individuals with Sickle Cell DiseaseValid driver s license and reliable transportationBilingual in English and Spanish requiredAbout UsTitan Placement Group is a permanent placement healthcare recruiting firm that is bridging the gap between healthcare companies and high-quality candidates.
