RN / Registered Nurse - Home Health

NEW Registered Nurse (RN) Home Health Direct Hire Seeking a compassionate and experienced Registered Nurse to provide skilled home health nursing care. This role focuses on patient assessment, care planning, medication management, coordination with physicians and care teams, and patient/family education in the home setting. Key Responsibilities: Assess patient eligibility and needs for home health services Develop, implement, and revise individualized plans of care Provide skilled nursing care considering physical, emotional, cultural, and social factors Coordinate care with physicians, therapists, aides, and interdisciplinary teams Perform medication reconciliation and identify potential drug interactions or compliance issues Educate patients and caregivers on disease management, safety, and emergency care Complete accurate and timely documentation in the EMR Conduct Home Health Aide supervisory visits every 14 days Support quality assurance, PDGM case conferencing, and performance improvement initiatives Ensure infection control and agency policies are followed Qualifications: Active RN license (multi-state or state-specific) Home Health nursing experience required Strong clinical judgment, communication, and organizational skills Basic computer proficiency and EMR documentation experience Valid driver s license, reliable vehicle, and acceptable driving record Position Details: Direct Hire Full-Time Schedule: Monday Friday, Day Shift (flexible to patient needs) Pay Range: $33 $46 per hour Mileage reimbursement: $0.70/mile Sign-On Bonus: $10,000 Interested!?
