Recovery Oriented Practitioner

We are seeking a professional with a Master's Degree in Human Services or related field to join our team as a Community Support Specialist III. The ideal candidate will have 2-5 years of experience working with adult populations, particularly those with complex needs. They must possess excellent communication and interpersonal skills, be able to work independently and collaboratively as part of a team. Key ResponsibilitiesProvide community support services to maximize opportunities available to individuals living in the community who are trying to recover from the serious and persistent effects of mental illness.Contacting individuals and/or referral sources following missed appointments in order to re-engage and promote recovery/resiliency efforts. Supporting individuals in crisis situations.A Master's Degree is required for this role.The successful candidate will also require 2-5 years experience dealing with clients on care plans using treatment plan interventions that result positive outcomes based individual strengths need; case load sizes vary.
