Travel Nurse Practitioner, Hybrid In-Home Chronic Kidney Care Richmond, VA We are seeking a Nurse Practitioner or Physician Assistant to provide in-home care for patients with Chronic Kidney Disease (CKD) through End-Stage Renal Disease (ESRD) in a progressive, outcomes-driven model that blends home visits with telehealth support. This is high-impact medicine - without weekends, call, or burnout scheduling. Why This Role Stands Out ? Monday-Friday schedule - no weekends, no call ? What You'll Do Conduct comprehensive initial evaluations and follow-ups Lead hospital-to-home transitions to reduce readmissions Perform thorough head-to-toe assessments and medication management Educate patients on CKD/ESRD disease progression and treatment plans Collaborate closely with an RN, Social Worker, and supervising physician Help prevent avoidable hospitalizations through proactive intervention Visit Structure: 90 minutes for new patients 30-60 minutes for follow-ups You'll have the time and support to practice thoughtfully - not reactively.
