Schedule:Full-Time 40 hours/weekMonday Friday 8:00 AM 5:00 PMNo nights. No weekends. No holidays. No on-call.Locations: Davidson & Guilford countiesProgram:Senior Community Care (SCC) / Institutional Special Needs Plan (ISNP)Employment Type:Potential Temp-to-HireOverview:Senior Community Care (SCC) program is a longitudinal, integrated care model designed to provide proactive, coordinated care to some of our highest-risk members residing in facility settings. As an Advanced Practice Clinician (APC), you ll work as part of an interdisciplinary care team delivering high-quality, patient-centered care while maintaining a true work-life balance.Why This Role:Consistent weekday schedule (8 5)No on-call, weekends, or holidaysFlexible schedulingMeaningful, relationship-based careStrong interdisciplinary team supportPrimary Responsibilities:Conduct comprehensive medical assessmentsManage acute and chronic medical and behavioral health conditionsEnsure accurate, complete documentation including ICD-10 codingCollaborate with interdisciplinary care team membersLead advanced illness and advance care planning discussionsProvide patient and caregiver education and counselingRequired Qualifications:Active, unrestricted NP or PA license in state of residence (or ability to obtain by start date)Graduate of accredited NP or PA programNational board certification:NP: AANP or ANCC (FNP, ANP, GNP, or AGACNP)PA: NCCPAActive DEA / prescriptive authorityAbility to meet physical requirements (lifting up to 30 lbs, navigating stairs, standing, kneeling)Reliable transportationAbility to obtain collaborative practice agreement if required by stateFull COVID-19 vaccination (per federal, state, and client requirements)Ability to pass pre-employment drug screeningPreferred Qualifications:1+ year of clinical experience (long-term care preferred)Experience managing patients with complex medical, behavioral, social, or functional needsKnowledge of geriatrics and chronic illnessComfort with advanced illness and end-of-life conversationsProficiency with EMR systems
