Facility in Adell, WI seeking In-Home Primary Care NP locums coverage Rates: $110/hr Opportunity Highlights Contract Term: 13-week initial assignment with month-to-month extensions. Guaranteed Pay: Paid for a full day regardless of patient volume (provided availability is logged in Salesforce). Schedule Flexibility: Full-Time: 40 hours/week (Mon Fri, 8 am 5 pm). Part-Time: 20 hours/week options available. No Call/Weekends: Enjoy a predictable daytime schedule with no on-call requirements. Clinical Mix: 70% Telehealth / 30% In-Home visits. Support: Mileage is reimbursed at the IRS standard rate for home visits. Clinical Role & Responsibilities This is an assessment-only role. You will focus on identifying clinical needs and chronic conditions to ensure patients receive proper follow-up from their Primary Care Providers (PCP). Daily Volume: Conduct 5 6 comprehensive assessments per day. Focus Areas: Annual risk assessments, chronic condition reviews, and social determinant screenings. Diagnostics: A1C testing only (if applicable); no other invasive screenings are required. Care Coordination: Identify and escalate urgent findings directly to the patient s PCP. Prescribing: None. This role does not require prescribing or active treatment plans. Provider RequirementsBoard Certification: BC Family Nurse Practitioner (FNP) (Required). Licensure: Active Wisconsin (WI) Medical License (Required). Medicaid Enrollment: Must be currently enrolled/credentialed in Medicaid. Please confirm status at the time of submission.
