Hospice/Palliative Medicine Nurse Practitioner

Position OverviewMonogram Health is seeking an experienced Palliative Care Nurse Practitioner to deliver in-home care to a highly complex patient population in the Newark, NJ market.This is a permanent, full-time position focused on end-of-life and serious illness care.
