The hospital medicine advanced practice clinician (APC) provides high-quality, patient-centered medical care to hospitalized adult patients in collaboration with hospitalist physicians and the interdisciplinary care team. The APC is responsible for evaluation, diagnosis, treatment, and ongoing management of acute and chronic medical conditions in the inpatient setting, ensuring safe, efficient, and evidence-based care.About the RolePerform comprehensive history and physical examinations for admitted patientsManage acute and chronic medical conditions in hospitalized adultsDevelop, implement, and adjust treatment plans in collaboration with supervising physiciansOrder, interpret, and act on diagnostic tests (labs, imaging, EKGs, etc.)Perform procedures as credentialed Respond to clinical deterioration, rapid responses, and medical emergencies as requiredParticipate in admissions, daily rounding, transfers, and dischargesConduct medication reconciliation and ensure safe prescribing practicesCollaborate with physicians, nursing staff, case management, pharmacy, therapy services, and consultantsCommunicate effectively with patients and families regarding diagnoses, treatment plans, and goals of careParticipate in multidisciplinary rounds and discharge planningProvide thorough handoffs during transitions of careCompetitive compensation with an estimated base salary range of $75 to $85 hourly with the opportunity to earn incentives.
