Nurse Practitioner - Hybrid Kidney Care Redefine how care is delivered for patients living with ESKD. We're seeking an NP who is deeply passionate about caring for individuals with End-Stage Kidney Disease (ESKD) and thrives in a hybrid care model-blending in-person clinical visits with meaningful telehealth connections. Why This Role Stands Out Hybrid Flexibility: A mix of in-clinic visits and remote telehealth care High-Impact Work: Support patients navigating complex kidney disease and comorbidities Autonomy + Collaboration: Lead care while partnering with nephrologists and interdisciplinary teams No Nights or Weekends: Monday-Friday schedule with built-in flexibility Your Impact You'll serve as a key clinical leader, guiding patients through their kidney care journey with a whole-person approach. Core Responsibilities: Conduct Comprehensive Health Evaluations (CHEs) addressing medical, social, and emotional needs Manage patients with CKD and ESKD, focusing on slowing progression and improving outcomes Provide medication management, diagnostics, and care coordination Reduce hospitalizations through proactive, relationship-driven care Deliver care across dialysis clinics, nephrology practices, and telehealth platforms Collaborate closely with interdisciplinary teams to align care plans Hybrid Work Model Telehealth (Remote): Conduct virtual visits, care planning, and follow-ups from your home office In-Person: Travel within the Sacramento/San Francisco region for clinic-based patient care Travel Expectation: 1x/month to Berkeley, plus local travel as needed What You Bring Passion for ESKD and complex chronic care populations Minimum 2 years of Nurse Practitioner experience Strong clinical judgment and comfort working autonomously Active NP license + DEA (or ability to obtain within 90 days) Current BLS/CPR certification Reliable transportation + ability to travel locally Tech-savvy with EHRs and telehealth platforms Compensation/Benefits: Salary: $140,000 - $155,000 annually Comprehensive benefits (medical, dental, vision, 401k match, PTO) CEU/CME support + license reimbursement Career development and leadership growth opportunities Resources supporting well-being, family care, and work-life balance You'll be part of a mission-driven team delivering proactive, patient-centered kidney care-helping patients live better, avoid hospitalizations, and feel supported every step of the way.
