On Call Licensed Vocational Nurse (LVN) - Hospice

Bella Terra Hospice Pay Range: $35.00 $40.00/hr Schedule: Nights 5:00pm 8:00am | On-call About Us Bella Terra Hospice is part of the Cornerstone Group, a network of over 75 home health and hospice agencies nationwide. What You'll Do As an On Call LVN, you'll play a vital role in supporting patients during nighttime hours. You'll work closely with RNs and physicians to ensure continuity of care and comfort for our patients. Key Responsibilities: Follow and implement the patient's plan of care as directed by the RN Provide timely and accurate documentation Conduct assessments and report findings to the care team Collaborate with interdisciplinary team members Use medical equipment and supplies efficiently Engage in ongoing professional development What We're Looking For Valid California LVN license Minimum 2 years of direct patient care experience At least 1 year of hospice experience preferred Current CPR certification Reliable transportation with valid driver's license and insurance Why Bella Terra?
