Position Summary: We are seeking a dedicated and compassionate Physician Assistant (PA) to join a skilled interdisciplinary healthcare team in a Subacute/Skilled Nursing Facility setting. The PA will be responsible for comprehensive patient care, including evaluation, diagnosis, treatment planning, and follow-up management under the supervision of a licensed physician. The role is ideal for professionals passionate about geriatric, post-acute, and rehabilitation care. Key Responsibilities : Conduct patient assessments and physical exams to diagnose and manage acute and chronic conditions. Collaborate closely with physicians and interdisciplinary staff to develop and implement care plans. Order and interpret diagnostic tests and therapeutic procedures. Prescribe medications within scope of license and monitor patient response to treatment. Provide patient and family education focused on recovery, rehabilitation, and safe transitions of care. Document all encounters in compliance with clinical and regulatory standards. Participate in team meetings, care planning, and continuous improvement initiatives. Required Qualifications : Must have 0-1 years of experience in Graduate of an accredited Physician Assistant program Must have 0-1 years of experience in Certification by the National Commission on Certification of Physician Assistants (NCCPA).
