Job Description AHRC NYC is seeking a Team Lead for their specialty Critical Time Intervention (CTI) Program. CTI is a highly focused, structured, time-limited intensive case management program that assists clients during 'critical transition periods' from hospital to community-based care. AHRC's CTI Team stands out in that it is the only one in NYC whose focus is on clients with co-occurring mental health and intellectual/developmental disability (I/DD) diagnoses. This program is primarily community and field-based work. In addition to the Team Lead, AHRC's CTI Team is comprised of one Care Manager, one Behavior Intervention Specialist, and one half-time Nurse. The team carries a caseload of approximately 30 individuals over the course of the year. The Team Lead will be directly supervised by the Enhanced Step-Down Program Director. Job Description The CTI Team Lead is a licensed mental health professional who plays a pivotal role in helping individuals successfully transition and thrive in the community. This role leads the delivery of Critical Time Intervention (CTI) services by building strong connections to community-based supports, addressing barriers to stability, and promoting long-term engagement, recovery, and social integration. The CTI Team Lead provides day-to-day clinical and programmatic leadership, monitors participant progress, and ensures timely problem-solving when challenges arise. They facilitate weekly team meetings, support staff development, and ensure fidelity to CTI model standards. The ideal candidate brings experience in OPWDD and OMH-regulated case management services and a passion for leading teams that deliver high-quality, person-centered care during critical life transitions. This position requires working in the field/ community as needed. LCSW, LMHC strongly preferred. LMSWs with extensive experience may be considered for the position. SALARY : $100,000 per year plus a comprehensive Benefit package...see Benefit information below. ESSENTIAL RESPONSIBILITIES: Comfort and familiarity with field and community-based work Carry a small caseload of clients Participation in evening and weekend on-call rotation, as needed Setting up systems for the team and ensuring that all team members are trained on the CTI model Conduct weekly team meetings Coordinate weekly case presentations Ensure continuity between fieldwork and meetings Ensure that entire caseloads are reviewed at minimum once a month Promote fidelity to the CTI model and a trauma informed care approach Monitor quality of documentation Update resource lists Monitor phase dates Manage the weighted caseload for CTI staff Ensure that data is entered and reviewed into EHR and regularly reviewed in order to track team's performance and effectiveness Performs additional duties as needed Qualifications Licensed Mental Health Clinician Licensed Psychologist, LCSW, LMHC LMSWs with extensive experience may be considered Prior OMH (ACT, IMT, CTI) experience preferred Experience working with people with both I/DD and psychiatric disorders strongly preferred Must have ability to plan, organize, develop, and implement program policies and procedures in a timely and consistent manner. Ability to be patient, enthusiastic, and confident when interacting with people supported by the program, understanding the psychosocial and cognitive levels at which they are functioning. Valid Tri-State or New York State driver's license for more than one year, in good standing, preferred. Must be willing to travel within five boroughs of New York City.
